Description of the project

The Joint Action on Cardiovascular Diseases and Diabetes (JACARDI) brings together 21 European countries and 76 cross-sector partners to deliver 142 pilot projects implementing proven prevention and care practices for cardiovascular diseases and diabetes. Building on collaborations with the WHO and insights from previous EU initiatives, JACARDI will test solutions across the entire patient journey, from prevention and early detection to treatment, care, and self-management, aiming to improve health literacy, strengthen prevention and care pathways, enhance self-management and participation in the labour market, advance data quality and accessibility, and promote equitable access to healthcare services and information.

Role of IDIAP JGol and ESVIDA project

The Foundation University Institute for Primary Health Care Research Jordi Gol i Gurina (IDIAPJGol) represents Catalan Primary Care as Affiliated Entity (AE). It participates in work packages 4 (Sustainability) and 10 (Patients’ Self-management). Within work package 10, IDIAP JGol will implement a pilot project with called ESVIDA. The ESVIDA project aims to develop, implement, and evaluate an intervention programme based on a gamification-based application, focused on the patient and adapted to their personality profile, to promote and improve adherence to healthy lifestyle habits in adults who are overweight or obese in primary care. The gamification tool will be co-designed by primary care professionals and patients who are overweight or obese. The pilot aims to encourage self-management among primary care patients by increasing their health literacy, enhancing motivation by adapting health messages to their personality profile, and supporting the integration of healthy routines into their daily lives.

The participation of IDIAPJGol in the JACARDI project is led by professionals of the South Metropolitan and Penedès Research Support Unit (USR) of Catalan Institute of Health (ICS), the largest public primary healthcare service provider in Catalonia. In the JACARDI project, three Primary and Community Care Managements (GAPiCs) from the ICS are participating: Delta, Baix Llobregat, and Penedès.
